The military makes a big deal out of when a rifle goes missing, not to mention when a nuke disappears. In spite of the fact the program is designed to be “zero defect,” here are 7 examples of doomsday devices wandering off (including a few where they never came back):

Other countries 'lose' bombs, too!  ISIS Magazine: Terror Army Could Buy Nuclear Weapon From Pakistan
 
An article in the official magazine of ISIS claims that the terror army has the financial wherewithal to purchase a nuclear weapon, possibly from corrupt officials in Pakistan. 
 
If Iran obtains a nuclear weapon, it's going to cause proliferation throughout the Middle East, increasing the chance that ISIS can get a nuclear device.

In it he details some of the most absurd, disturbing, and woefully misguided ways we dealt with, or failed to deal with in many cases, the most destructive technology in the history of humankind. Plutonium was implanted in people’s hearts; reactors were shot into orbit, after which they rained down on Earth; 40 nuclear bombs were declared missing. There were even plans to bomb the moon.
